MODEL RELEASED. Blood donor questionnaire. Potential donor answering questions on his medical history. All donors are required to provide background information about their medical history as well as recent holidays, sexual practises and body tattooing & piercing. These questions, along with a sample of blood that is tested for disease, are checked before blood is accepted. Donor blood is given to patients in transfusions to replace lost blood, for example during surgical procedures or after accidents.
