Milan Italy isn't big on sites, but it's biggest site is BIG! It is Il Duomo, or the Metropolitan Cathedral-Basilica of the Nativity of Saint Mary. This was pretty much my first view of the cathedral as I exited the metro. It is definitely a WOW moment if you have never seen it before. The Gothic cathedral took nearly six centuries to complete. It is the second largest church in Italy after St. Peter's Basilica in Rome and the third (or fourth or fifth, depending on where you read or perhaps how you measure) in the world. Everyone goes there as you might think from the crowds waiting to get in. Its architecture has been praised and reviled, but you can't deny that it is an arresting sight.
